Treating you well since 1948 Bond Clinic is proud to be partnering with the Polk County Health Department for vaccine distribution. You may sign-up by visiting https://register.polk.health/ or by calling their call center at 863-298-7500. The call center is open Mon-Fri 8am-5pm and Sat-Sun 8am-2pm.
At this time, Bond Clinic providers do not have access to vaccinations and we encourage our patients to register with the county.
