What is Chronic Care Management?
Bond Clinic Quality Management Team works with you and your doctors to help meet your health goals. Create a personalized care plan and oversee it for at least 20 minutes per month via telephone. This program does not replace your visits to the doctor. It only enhances communication.
What is the Chronic Care Management Program?
It is a new Medicare program designed to keep you out of the hospital when possible. The program allows us time to take better care of you.
How does it work?
Our team will contact you by telephone to review a series of questions. This will help keep you on track to meet your health goals, which you and your doctors have set. The call will be documented in your medical chart for your Primary Care Physician to review.
Who determines if I am eligible for the Chronic Care Management Program?
Your physician will decide if you qualify for the new program. Your physician will speak with you about the program during your clinic visit and have you sign a consent form. You must have two or more Chronic diagnoses listed in the chart below.
Do I still see my doctor?
Yes. This does not replace regular doctor visits. However, this program may even decrease the frequency of your regular visits if that is what you would like.
How does it work?
It’s a VIP check-in service for patients with chronic diseases. A Nurse from our team will phone you every month to check on your progress and report the results to your doctor. Based on this information, your doctor can fine-tune your treatment, guiding you to better health.
What’s the benefit?
In the program, we will work closely with your doctor. You will also have 24-hour access to your healthcare team. Studies show similar programs can reduce unpleasant activities like going to the emergency room and getting admitted to the hospital.
I’m seeing more than one Bond Clinic doctor. Do all of them offer this service?
Only one primary care provider can bill for this service. If you see physicians at Bond Clinic, we can review your records. You can cancel the service at any time, but we hope you will take advantage of this benefit, which allows us to take better care of you.
